Travel Forms
All of these forms will be available in late May and will include a packet of what is needed - just print out the forms that are needed. Here is a checklist of what you will need to send in to Camp:
- Health Form (can be found under Required Camper Forms)
- Daily Medication Form (can be found under Required Camper Forms)
- Recent photo of your camper (remember to write his/her name on back of photo)
- Camper Contract (signed)
- Rainy Day Contact Form
- Flow Rider release form for Sahara Sam’s(if your camper is attending week 3)
- Sky Trail Course Arbitration Agreement for Sahara Sam’s (if your camper is attending week 3)
- Ice Skating release form for the Skate Zone (if your camper is attending week 6)
- Indoor Rock Climbing release form for Go Vertical (if your camper is attending week 7)
- Overnight Medication Form (if your camper is attending week 4 or week 8 extended trip)
